How does the Plus System (AKA “Plus Two System,” AKA “Plus 2 System”) work?

A visual alternative to the Plus System, with no diamond numbering or calculating, is the midpoint-parallel-shift system.
The Plus System is used mostly for aiming two-rail kicks off a short rail, but it can also be used to aim three-rail kicks off a long rail as demonstrated in the following video:
